We schedule our appointments over the phone and through our online scheduling portal.  The physicians of Prosper provide provide a team care approach. This means you are able to book with any of our physicians. This unique clinic setup allows your doctors at Prosper Natural Health to collaborate in your care as they meet weekly to discuss clinical cases. We provide you more availability to be seen by your trusted physicians and you benefit from the expertise of all of our doctors.

Before your first visit

1. Please complete the digital intake form emailed to you prior to your visit. It is the preferred, comprehensive intake method which will ensure a more productive visit with us.  If you are unable to complete the digital form, please download and complete the appropriate intake form below.

Adult Intake Form Please fill this form out if you are a new adult patient. (Patients over the age of 14 years old). 

Pediatric Intake Form New patient intake form for children 0-14 years of age.

PAP – Gynecological Exam Intake Form For new patients who are having a female wellness exam.

Notice of Privacy Practices This Privacy Practices notification should be reviewed, but does not need to be printed or brought in with you to your appointment.

If you are unable to complete the online intake forms or print these PDFs, please call our patient care coordinators at (360)385-5375, and we will send you an introductory packet by fax or mail. Otherwise, if you have an in-person initial appointment, please arrive 20-30 minutes early for your appointment to allow time to complete the necessary introductory paperwork.

2. If you are taking supplements or other medicines, please bring them to your first appointment.

3. If you have copies of your most recent lab work, please bring a copy to the visit.

4. Write down your questions and concerns prior to the visit to ensure that they are answered.

 

Your first visit

Initial appointments are typically between 60-90 minutes long. During this visit, you and your doctor will discuss your health concerns and you will be given the opportunity to share you medical history. Your doctor will perform a physical exam and, if necessary, discuss tests that would be useful in clarifying your condition and needs. At this visit, you will receive a customized treatment plan specifically for you. This plan is designed to help correct your underlying imbalances and to move you towards your optimal health.

Your return visits

Return visits are typically 30 minutes in length. These visits are designed as check-ups to see how you’re doing, note positive changes, and make adjustments to your personalized treatment plan. To best suite your needs, a physical exam maybe performed at this visit. Your doctor may recommend further testing, depending on your personal needs and condition.

Cancellation & missed appointments

We ask that you give us at least 24 hours notice if you need to cancel or reschedule your appointment. If you cancel your appointment without 24 hours notice or miss your appointment, you will be charged a $50 fee.

If you are an existing patient, the best way to contact our doctors regarding your treatment is by leaving a message through your patient portal or by talking with our staff during business hours at (360)385-5375. You may also leave a message on our confidential voicemail.

Email is not a dependable or secure form of communication for healthcare matters so our office asks that you communicate by phone or via the secure patient portal for all existing patient care matters.
