Scheduling appointments

For an appointment, please call (360)385-5375.

We schedule our appointments over the phone, rather than email due to the rapidly changing nature of our schedule. Email requests, although speedy, are often times not quick enough to secure the slot desired.

Before your first visit

1. Please download and complete the appropriate intake form.

Adult Intake Form Please fill this form out if you are a new adult patient. (Patients over the age of 14 years old). 

Pediatric Intake Form New patient intake form for children 0-14 years of age.

PAP – Gynecological Exam Intake Form For new patients who are having a female wellness exam.

Notice of Privacy Practices This Privacy Practices notification should be reviewed, but does not need to be printed or brought in with you to your appointment.

If you are unable to print these PDFs, please call our office manager at (360)385-5375, and we will send you an introductory packet by fax or mail. Otherwise, please arrive 20-30 minutes early for your appointment to allow time to complete the necessary paperwork.

2. If you are taking supplements or other medicines, please bring them to your first appointment.
3. If you have copies of your most recent lab work, please bring a copy to the visit.
4. Write down your questions and concerns prior to the visit to ensure that they are answered.

 

Your first visit

Initial appointments are typically between 60-90 minutes long. During this visit, you and your doctor will discuss your health concerns and you will be given the opportunity to share you medical history. Your doctor will perform a physical exam and, if necessary, discuss tests that would be useful in clarifying your condition and needs. At this visit, you will receive a customized treatment plan specifically for you. This plan is designed to help correct your underlying imbalances and to move you towards your optimal health.

Your return visits

Return visits are typically 30 minutes in length. These visits are designed as check-ups to see how you’re doing, note positive changes, and make adjustments to your personalized treatment plan. To best suite your needs, a physical exam maybe performed at this visit. Your doctor may recommend further testing, depending on your personal needs and condition.

Policies

Cancellation & missed appointments

We ask that you give us at least 24 hours notice if you need to cancel or reschedule your appointment. If you cancel your appointment without 24 hours notice or miss your appointment, you will be charged a $50 fee.

Email is not a dependable form of communication for healthcare matters so our office prefers to communicate by phone or in person for all existing patient care matters.

Payment

Payment is always due at time of service. We accept cash, checks, Visa, or MasterCard.

Insurance

To provide you with the highest quality of healthcare, Prosper Natural Health does not contract with insurance companies. The medical insurance system has created a healthcare system where the doctor-patient interaction is rushed and the insurance company dictates medical treatment. You will never experience this at Prosper Natural Health. This means that Dr. Molly does not bill insurance companies for the goods and services that you receive, and your payment is due at the time of service.

We are happy to provide you with the necessary codes and paperwork to submit to your insurance company for reimbursement. At the time of service, you may request a “superbill” which will have all the necessary coding information for your insurance carrier. Reimbursement rates vary, so please call your insurance provider to determine your coverage for a primary care physician who is an “out-of-network provider.”
